Scoring Status Guide

Please calculate your score based on the number of “YES” responses.

Note: Any “N/A” responses should be excluded from the total expected count (e.g., scoring 9/9 YES if one item is genuinely N/A would still be considered Green).

GREEN STATUS (10 YES)

Excellent. The facility is fully compliant and up-to-date with current 2026 regulations. Continue routine maintenance.

YELLOW STATUS (6 to 9 YES)

Caution. There are moderate compliance gaps. An action plan must be drafted within 14 days to resolve these deficiencies.

RED STATUS (0 to 5 YES)

Critical. The facility is at high risk of non-compliance and potential regulatory fines. Immediate corrective action and a mandatory review meeting are required.
